Query ID | In the text box, type the query ID and click Submit ( |
Patient ID | In the text box, type the patient ID and click Submit ( |
Site Name | In the text box, type the site name and click Submit ( |
Type | From the drop-down list, select the level of the query within the hierarchy:

Visit Name | From the drop-down list, select a specific visit or All. |
Form Name | From the drop-down list, select a specific form or All. |
Status | From the drop-down list, select a query status. Can include:

Site Name | Name of the site to which the patient belongs. |
Query ID | Unique identifier for the query. |
Patient ID | Identifier for the patient associated with the query. The application sorts the Patient ID in ascending order by creation date. |
Type | Level on which the query was created: visit, form, or item. |
Visit Name | Name of the visit. |
Form Name | Name of the form. |
Item Text | The text of the queried question. |
Query Text | Text of the query. |
Status | |
Issued by | User who created the query. The author is System if the query was automatically issued. |
Date | Date the query was created. |
