When you are working with a return order and there is a Gift with Purchase promotion applied to the returned item, note that unless your administrators have configured Commerce Service Center differently, Commerce Service Center will not remove the gift that is associated with the returned item from the cart, but will re-price the gift within the order. Depending on your company policy, you can manually re-price the returned item or the gift as necessary.

When you are working on an exchange order and there is a Gift with Purchase promotion applied to the exchanged item, you may experience differences depending on the type of gift. If there are multiple selections for a free gift, when you add the exchange item to the order, no free gift selectors will appear. However, if the free gift was a single SKU or product, there may be a Select Free Gift link in the exchange cart.
